Cayuga Nursing and Rehabilitation Center, a 144 bed facility, is seeking a Per Diem Occupational Therapist to work alongside our amazing staff in our recently renovated gym. The Occupational Therapist is responsible for evaluating residents and subsequently developing a treatment plan that will meet their needs with activities that will help them become more independent in their daily lives. The Occupational Therapist will observe and assess the progress of our residents and prepare detailed reports accordingly.

Looking for weekend coverage and coverage for OTs taking paid time off.

All Occupational Therapists must be a graduate of an accredited Occupational Therapy Program and have a current New York State Occupational Therapy license. Experience in a long term care setting is preferred, although not necessary. We encourage newly graduated Occupational Therapists to apply.

The Medical Records Department is responsible for the overall management, accuracy, compliance, and security of the medical records department in a long-term care setting. This role ensures timely and accurate documentation, supports clinical and financial processes, maintains regulatory compliance, and serves as a liaison with providers, payers, and internal departments.

Medical Records Secretary Job Responsibilities:

  • Prepare/maintain medical records for current, discharged, and newly admitted residents according to the checklist.
  • Assist in maintaining current physician scheduling, including podiatry, dental, and ophthalmology visit listings.
  • Upload and maintain external medical records and physician visit records
  • External appointments - scheduling new and follow up appointments, ensuring paperwork is complete, transportation is arranged, and communicate with nursing and other disciplines as needed.
  • Complete admission record review and checklist, 7 days post admission
  • Maintain HIPAA compliance
  • Prepare discharge packets for residents

Medical Records Secretary Skills & Qualifications:

  • Prior experience in medical records management in a Skilled Nursing Facility or Long-Term Care setting strongly preferred.
  • Working knowledge of PointClickCare (PCC) required or strongly preferred.
  • Knowledge of Medicare, Medicaid, PPS, and managed care requirements.
  • Familiarity with HIPAA regulations, chart audits, and QA processes.
  • Strong organizational skills with attention to detail.
  • Ability to manage multiple priorities in a fast-paced healthcare environment.
  • Strong communication skills and ability to collaborate with interdisciplinary teams.

Cayuga Nursing and Rehabilitation Center is actively seeking a motivated and experienced Recreation Leader to work at our skilled nursing facility in Ithaca, NY. The ideal candidate should have experience in long term care, be well organized with excellent communication skills, and have familiarity and true enjoyment from working with the needs of the elderly. Evening hours preferred with weekend availability needed.

Job Duties:

  • Engage new residents and determine their interests
  • Assist in running recreational programs to meet the interests of the residents
  • Assist volunteers and their activities
  • Engage, interact, and actively participate with residents at programs
  • Assist residents with 1:1 support at programs to achieve highest level of participation
  • Regularly document resident's participation in/at programs and 1:1 visits on a daily basis
  • Engage, interact, and actively participate with residents at programs

Requirements:

  • Previous experience working with seniors and/or in a healthcare setting is a plus

We are seeking a Physical Therapist Assistant (4month temp position, 40hours per week from April-July) with the opportunity to remain Per Diem. We are looking for an experienced or newly graduated Physical Therapist Assistant to join our team. The Physical Therapist Assistant is responsible for the treatment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Physical Therapist Assistant will administer a treatment program with specific goals determined according to the patient's capacity. As well, the Physical Therapist Assistant must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Requirements include:

Must have graduated from an accredited school. Current state license as a Physical Therapist Assistant or proactively in the process of the application process for current state licensure as a Physical Therapist Assistant .Current CPR certification. Responsibilities

  • Assists in planning and implementing the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse

  • Ability to adequately monitor, treat and report resident pain whether physical or emotional, verbal or nonverbal. Demonstrates knowledge of symptoms of pain and depression.
  • Utilizes appropriate pain management techniques checks for available prn’s and administers appropriately as well as monitors for and documents/reports effectiveness.
  • Administers medications safely and per policy, follows the medication rights and reduces the potential for medication errors. Demonstrates competency in medication administration per regulations. Utilizes prn medication when needed.

  • Valid New York State Licensed Practical Nurse (LPN) Certification
  • 1 or more year of experience is preferred

We are looking for an experienced or newly graduated Physical Therapist Assistant to join our team of experienced Physical Therapists.   • Consulting with patients to learn about their physical condition and symptoms after reviewing their medical history
  • Develop an individualized treatment plan, tailored to the specific goals of the resident made by the Physical Therapist
  • Instructing and counseling patients using a variety of treatment techniques including: Manual therapy, Therapeutic exercises, Modalities, Gait training, ect.
  • Maintain patient records by keeping track of goals and progress; adapting treatment plans accordingly
  • Communicate effectively with patients and their families on what to expect from the recovery process
  • Communicating with IDT on patient’s needs, to maximize outcomes.

PTA Requirements:

  • A minimum of an Associates degree in Physical Therapy from an accredited program
  • Has current PTA license (or is license-eligible) in New York State

**$2000 Sign on Bonus Available** We are looking for a Certified Occupational Therapy Assistant to work under the direction and supervision of Occupational Therapists as well as the Director of Therapy.

Typical work duties: Certified Occupational Therapy Assistants typically do the following:

  • Provide comprehensive treatment to patients under the supervision of an OTR and according to state guidelines
  • Document patient’s progress on weekly progress note in an accurate and timely fashion
  • Attend facility and rehab meetings when indicated by supervisor
  • Complete billing logs and bill patients ethically and accurately for occupational therapy services rendered
  • Cover other therapists’ patient caseload during absence as necessary
  • Maintain professional relationship with co-workers, facility staff, patients and patient families
  • Provide inservice education regarding patient care or occupational therapy services to rehab staff, facility staff, hospitals and/or community as needed
  • Comply with patient confidentiality and Federal Resident Rights
  • Report to work on time, adhere to scheduled hours, and project a professional image at all times
  • Adhere to productivity requirements, complete productivity reports and submit in a timely manner
  • Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers
  • Perform all other duties assigned by the department supervisor, lead therapist or regional manager

Requirements:

  • A minimum of an Associates degree in Occupational Therapy from an accredited program
  • Successful passage of the National Board of Certified Occupational Therapists (NBCOT) COTA examination
  • Has current OTA license (or is license-eligible) in state of practice
